Applicants for both the Freshman class and transfer students must complete the following.  If you are transferring from another high school, please contact the Admission’s office before filling out an application:

  1. CRSM Online Admissions & Finance Application (requires e-mail address)
  2. Applicant Birth Certificate
  3. Application Non-Refundable Fee $25
    Forms of Payment Accepted: Debit / Credit Card and Banking Account
  4. 7th Grade Report Card or High School Transcript
  5. Current Report Card: 1st and 2nd Quarter Grades
  6. Copy of Standardized Test Scores (PARCC, MAP, Terra Nova or ASPIRE)
  7. Attendance Report
  8. Consent to Release Form
  9. Recommendation Letters
    1. Math Teacher Recommendation Form
    2. English Teacher Recommendation Form
    3. Principal / Counselor Recommendation Form
  10. Student Discipline Report
  11. Applicant’s Essay
  12. CWSP Training & Summer Bridge/Enrichment Courses Acknowledgment
  13. A copy of parents’ current tax forms (W2s and 1040 including all forms and schedules)

When all of the above documents are submitted we will review your file for initial eligibility. Please refer to our Admissions Calendar(Calendario de Admisiones) for important dates. Qualified students will then be invited, along with their parents, to interview. Interviews are held during December and February.

The Admissions Committee meets soon afterwards to review applications and determine final admission. We mail decision letters by the end of the month following the interviews.

Once the student is admitted, the following forms will be supplied to the student and must be completed before registration is complete:

  4. St. Mary’s University of Minnesota Informed Consent / Medical History
  5. St. Mary’s University of Minnesota Participant Agreement, Release and Consent for Emergency Treatment
  6. 2018-2019 Student Handbook Acknowledgement Form
  7. Student Health History Form
  8. Medication Consent Form
  9. Permission Form for Counseling Services
  10. State of Illinois Child Health Examination (Most Recent)
  11. Immunization Record
  12. Complete the Online Employment Eligibility Verification Form I-9 & the Employee Withholding Allowance Form W- 
4 (dates & times will be provided on your acceptance letter)
  13. A Non-Refundable Registration Fee of $50 will be automatically withdrawn by the Finance Department

All incoming students are required to attend the mandatory seven-week summer Bridge program and the CWSP institute.  Additionally, all incoming students will need to provide appropriate documentation so that he/she can complete an I-9 Form and an Illinois Department of Labor Employment Certification Form.
