Applicants for both the Freshman class and transfer students must complete the following.  If you are transferring from another high school, please contact the Admission’s office before filling out an application:

  3. Application Non-Refundable Fee $25. Forms of Payment Accepted:

  4. Picture of the Applicant (picture will be taken in the office)
  5. 7th Grade Report Card or High School Transcript
  6. Current Report Card: 1st and 2nd Quarter Grades
  7. Consent to Release Form
  8. Copy of Standardized Test Scores (PARCC, MAP, Terra Nova or Image)
  9. Recommendation Letters
    1. Math Teacher Recommendation Form
    2. English Teacher Recommendation Form
    3. Principal / Counselor Recommendation Form
  10. Student Discipline Report
  11. Applicant’s Essay*
  12. Parent’s current W-2 and 1040 Tax Forms

When all of the above documents are submitted we will review your file for initial eligibility. Please refer to our Admissions Calendar(Calendario de Admisiones) for important dates. Qualified students will then be invited, along with their parents, to interview. Interviews are held during December and February.

The Admissions Committee meets soon afterwards to review applications and determine final admission. We mail decision letters by the end of the month following the interviews.

Once the student is admitted, the following forms will be supplied to the student and must be completed before registration is complete:

  8. Permission Form for Counseling Services
  9. State of Illinois Child Health Examination Form
  10. 60 Second Survey (Admission Department)
  11. W-4 & I-9 for Employment Purposes
  12. $50 Registration Fee

All incoming students are required to attend the mandatory seven-week summer Bridge program and the CWSP institute.  Additionally, all incoming students will need to provide appropriate documentation so that he/she can complete an I-9 Form and an Illinois Department of Labor Employment Certification Form.
